*REPEAT VIOLATION* In the rear storage room, two dead cockroaches (1 adult, 1 nymph). No other vermin evidence observed after a thorough inspection of the facility. Per PIC, pest control service occurs every 2 months. Pest control report from 03/15/2025 provided and report did not state vermin activity. [CA]: Ensure food and food contact surfaces are protected from possible contamination at all times. Continue to follow pest control service recommendations and monitor for vermin activity. Seal all cracks, crevices, and gaps under any exterior doors to prevent entry and harborage of vermin. [COS]: Staff removed dead cockroaches.
