Follow up inspection due to repeated vermin major violation. Evidence of active vermin activity observed at time of inspection. New rodent droppings observed in back area and on top of boxes of equipment and near food contact equipment.
1. Observed Vermin: Documented in the following areas: Rodent droppings still observed on wooden beam running through facility along sides. Rodent droppings also observed on top of boxes near cooking and food equipment. One dead cockroach observed on the floor under shelves. 2. Photographs: Taken for documentation purposes. 3. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. Facility operates as auxiliary storage for Panda Express building near by. Facility shall not use any food contact equipment or food stored inside this storage area. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Panda Express may operate and store foods and food contact equipment in main store or other permitted food facility near by in Great America temporary. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.
ROUTINE INSPECTION
REPEAT VIOLATION: Rodent dropping observed behind shelves on beam. Per operator pest control come regularly and will be by to add traps and doors were sealed recently. [CA] Clean and sanitize area of dead cockroaches or old droppings. Increase frequency of pest control and monitor for signs of new vermin activity. [COS] Rodent droppings were removed and areas were sanitized. Follow up inspection will be conducted in July due to repeated violation.

Rodent droppings observed on ledge behind freezer and behind equipment storage shelf. [CA] Clean and sanitize area of dead cockroaches or old droppings. Ensure to routinely monitor for new vermin activity. Seal any vermin entries and remove any attractants.
Grate observed to be missing from floor drain. [CA] Provide grate for floor drain.
