    In the back prep area, observed standing water below the soft serve machines. At the front counter, observed food debris around edges outside of cabinets and on the floor below the trash container drawer. At the front counter, on the floor coving and walls behind the single door prep unit, observed cracks and crevices. [CA] Ensure facility floors are frequently cleaned and maintained to prevent harborage of vermin. Seal all cracks and crevices to prevent possible harborage of vermin.

    In the cabinet below the handwash station, observed several dead cockroaches. In the back prep area, below the soft serve machines, observed dead cockroaches. [CA] Ensure facility is kept free of cockroach activity. Clean and sanitize all areas with activity. Contact pest control to provide service as necessary. Seal all cracks and crevices to prevent possible harborage of vermin.

  • K29: Toxic substances properly identified, stored, used

    At the 3 compartment sink, measured chlorine sanitizer at 200 PPM. [CA] Ensure chlorine sanitizer used for manual warewashing is maintained at 100 PPM.

FOLLOW-UP INSPECTION

Upon 2nd follow-up inspection, found no LIVE cockroaches. Pest control services came out on 8/14/25 and 8/19/25. Per operator, pest control company will continue to provide services once a week until further notice. Facility is hereby reopen and okay to operate. Supervisor notified: Priscilla Mark. Facility shall be invoiced FP97: $298.00 for 2nd re-inspection.

FOLLOW-UP INSPECTION

Follow-up inspection for closure due to cockroach activity. Observed several live cockroaches in the facility at the following areas: 1. Behind the prep sink (in the crevice between the wall and the sink) 2. On the wall to the right of the 2 door reach in refrigerator (wall has peeling paint and oven holes) 3. On the floor around the grease trap cover and in the crevices underneath the cover. Continue to clean and sanitize all areas with activity. Seal all cracks and crevices and finish walls and the grease trap cover/grease trap opening to prevent possible harborage of vermin. Contact pest control to provide additional service for the facility. Use monitoring traps in the facility to evaluate areas for activity. The next inspection will be charged at a rate of $298.00 per hour. Any after-hours or weekend inspections will be charged at a rate of $645.00 (two hour minimum).     Observed three live young cockroaches in the coffee preparation area in cabinetry under espresso machine. Also saw an adult cockroach appear from the doorframe from back kitchen space. FACILITY SHALL REMAIN CLOSED FOR BUSINESS DUE TO PRESENCE OF LIVE VERMIN. FACILITY SHALL NOT REOPEN UNTIL AFTER AN INSPECTION TO VERIFY COMPLIANCE AND RECEIVING A FORMAL WRITTEN NOTIFICATION TO REPOPEN.
