    Measured fresh mozzarella at 51 F within over filled containers at the sandwich prep fridge. Observed all items within the upper portion of the unit to be inserted into ice baths. PIC stated that the upper portion of the unit is non-functional. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Top portion of the fresh mozzarella cheese relocated to be cooled to and held at 41 F or below. Rearrange all PHF's within the top of the unit so that they are fully inserted into the ice baths.

    Hot water handle on mop sink missing. [CA] Replace missing handle on mop sink.

    Several items placed in ice baths measured above 44F for less than 2 hours. Cut cherry tomatoes and sprouts in ice bath in broken unit. Sour cream in ice bath on counter near Doordash computer. [CA] PHFs shall be held at 41°F. [COS] Operator added more ice and water to go further up the sides of containers.

Air curtains shall be in operation over all open exterior doors to prevent flying pests. Observed the use of a fly strip above the janitorial sink. [CA] Insect control devices that are used to catch flying insects shall be designed to retain the insect within the device and be installed away from the storage and preparation of food or food contact surfaces and linens.
