ROUTINE INSPECTION

A limited inspection was conducted after live cockroach activity was notice shortly after entering the facility. - Facility is hereby closed due to evidence of a vermin infestation. - Facility shall: - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Eradicate all live and dead rodents and insects from the facility. - Clean and sanitize the affected area(s) and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal). ** Please note that pest control treatments for rodents or insects may require a sufficient amount of contact time for these to be effective.     Raw chicken and raw beef were found out of temperature control and measured at 60F near the cookline. Per PIC, they were removed from the cooler about an hour prior. [CA] Potentially hazardous foods shall be held at or below 41°F or at or above 135°F. [COS] Items were moved back to the cooler.

[CA] Maintain the floors, walls, and ceilings of the facility in good repair and clean. Repair the wall tile. Seal the gaps under the counter tops.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: On a Roll NEW OWNER: MyMy Fruits and Foods LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. New Owner will take over on 3/28/2026. The permit category for this facility is FP11. An invoice for the permit fee in the amount of $2281.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. This is the first follow up inspection: Observations: K08: All food on TPHC were time marked appropirately. K23: No evidence of cockroaches in the facility were observed. Facility has sealed gaps under the counter and around pipings under the three compartment sink and prep sink. The tiles are in process of being replaced. Ensure to clean wire shelfing of grease residue. Reviewed pest control report. Report dated for 9/24/2025 indicated no evidence of vermin observed. Ensure to correct identified areas of concern that the pest control report cites. Continue to correct any and all pending violations.

    In the 3 door preparation refrigerator: bbg pork, grilled chicken, and grilled pork were observed cooling in deep metal containers. [CA] Properly cool PHF's using shallow containers, using no more than 2 inches for solid foods and no more than 4 inches for liquids. After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan.
