Facility is using cardboard to line shelves. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

There are several holes in the ceiling. Note: Per facility manager, Dustin, they are going through a pipe replacement process. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

Hot water at the mop sink measured at 82F. Note: The handwash sink and the two compartment sink measured at least 120F. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks.

The only handwash station lacked paper towels in the dispenser. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] PIC restocked the dispenser.
