This is a second follow up inspection. Reviewed completed check list and pest control service report prior to inspection. Pest control report stated 'no activity found at time of service.' No evidence of live or dead cockroaches noted at time of inspection. Facility has been re-opened and ok to operate. Continue to correct all minor violations from previous inspection report. An invoice for todays second follow up inspection (charged at $298.00/hr, minimum of one hour) will be billed to billing address on file.

FOLLOW-UP INSPECTION

This is the first follow up inspection after a cockroach closure. A completed checklist and pest control report was provided prior to the follow up inspection. Observations: K23: A live cockroach was found on the instant water heater below the hand wash station. Cockroach droppings were observed on the electrical outlet near the instant water heater. K45: The cove base under the three compartment sink was repaired. The wall under the hand sink was repaired. The broken floor tiles under the cookline equipment was repaired. Facility shall remain closed until authorized to reopen by this department. The first follow up inspection is free of charge.
