Observed a spray bottle with chemicals unlabeled. [CA] Label all spray bottles with the contents. [COS] The bottle was labeled.
- K45: Floor, walls, ceilings: built,maintained, clean
The floor has several broken tiles and it is causing standing water in the cracks. The walls have mold in some spots. [CA] Thoroughly remove the mold and repair the floor and walls. The ceiling vent in the dish machine area has a small leak. There is currently a maintenance person working on correction of this leak. [CA] Repair and take effective measures to protect food and dishes/utensils from contamination.
- K23: No rodents, insects, birds, or animals
Observed a dead cockroach on the floor near the hood. [CA] Take effective measures to eliminate insects from inside the facility. The employee discarded the insect and cleaned the area. There is an active pest control company.
