OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: LHCF - Los Altos Post-Acute NEW OWNER: TURKIS HOLDINGS LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. An invoice for the permit fee in the amount of $1,148.00 plus a 25% late fee of $287.00, for a total of $1,435.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 1/1/2026 - 12/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 12/15/2025.

The following Potentially Hazardous Foods (PHFs) were measured at improper holding temperatures : Held inside the walk in, tuna salad (47F), cooked eggs (47F). Manager stated items were taken out the walk for morning sandwiches. Walk in ambient measured at 43F. [CA] PHFs shall be held at 41°F or below or at 135°F or above.
