Observed glue boards that are full of roaches and insects. [CA] Discard these and replace with new. Take effective measures to eliminate the roaches from inside the facility. Continue to keep food protected from contamination. Continue with the pest control service. Cover all holes in the walls. Remove items from the walls that are not needed and have a gap behind them as this is harborage for the insects to go behind.

There is a gap under the back door that leads to outside. [CA] Close the gap to keep out insects.

There are spaces between a couple of the hood vent slats from it being bent or missing some. [CA] Close the gap in the hood vents.
- K45: Floor, walls, ceilings: built,maintained, clean
Observed some holes in walls and gaps around pipes that go into the walls. [CA] Repair the walls. Close the holes and gaps in the walls as this will help keep out the insects. The floor tile grout is missing in several areas causing accumulation in between the tiles. [CA] Repair the tile floors and walls where broken and redo the grout where missing.
