ROUTINE INSPECTION

Note: 1. This is a second follow-up inspection for the routine inspection that was conducted on 07/29/25. The facility was issued for the lack of paper towel on the restrooms and for failing to log time and temperature for food products stored outside the safe temperature zone. During the first follow-up inspection, in addition to lack of time/temperature logs, employee was observed washing dishes without sanitizer. Therefore, the facility was issued a conditional pass on both occasions. During today's follow-up inspection, both violations were observed to be corrected. Therefore, the facility is issued a green placard. 2. Since this is a second follow-up inspection, the owner shall be billed $298.00 a second follow-up inspection fee.

FOLLOW-UP INSPECTION

Note: This is a follow-up inspection to the routine inspection conducted on 07/29/2025. The restaurant was issued a conditional pass due to the lake time/temperature logs for the food products stored at room temperature and for lack of paper towel supply on the restrooms. During today's follow-up inspection, the restrooms have soap and paper towel supplies. However, some of the food products stored on the steam table which were measured below 135 oF did not have any time/temperature logs. Employee was also observed washing dishes without any sanitizer. Therefore, the facility is issued another conditional pass. Please fix the issues and contact the district inspector within three business days for a follow-up inspection. Since the upcoming follow-up inspection is going to be a second one, the owner shall be a charge of $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645, minimum two hours, during non-business hours,
