    Observed black mold like substance on inside panel of ice machine. [CA] Thoroughly clean and sanitize ice machine on a regular basis.

    Observed potentially hazardous food that measured in the temperature danger zone located in a cambro warmer (just brought from the neighboring kitchen facility); the burrito measured at 103F. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. [SA] PIC will time mark the food for 4 hours starting from when the food was removed from hot holding and will discard at the 4 hour mark.

    Observed approximately 100-200 dead cockroaches of various life stages throughout the kitchen; particularly concentrated in the warewash room. Observed cockroach feces on the walls and ceilings. [CA] Remove dead cockroach bodies and thoroughly clean and sanitize walls. Provide pest control reports to the inspector by 8/26/2025.
