Paper towels at the hand wash sink in the back area maintained on top of dispenser. Hand soap dispenser was also located at the three-compartment sink. [CA] Paper towels shall be stored inside the dispenser to prevent potential contamination. Ensure paper towels and hand soap are easily accessible and readily available at all times. Hand wash station inside the restroom lacked paper towel dispenser. Towels were maintained inside a basket next to the sink. [CA] Provide paper towel dispenser.

Numerous prepackaged food items for customer self-service lacked labels and/or complete labels. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Per PIC, the following food items maintained inside the chest freezer were supplied from a home: - 16 large rolls (approximately 1 lb each) of raw fermented pork. - 9 banana and jackfruit ice cream bars. - 5 to-go containers of frozen tapioca dumplings. - 6 bags (ziplock and red mesh bags) with approximately 5-6 1-inch by 1-inch individually wrapped raw fermented pork in banana leaves. - 2 bags with approximately 10-15 individual servings of steamed egg cake 6 gallon-sized ziplock bags of raw eggrolls maintained inside the upright freezers, per PIC were also supplied from a home vendor. [CA] Food must be from an approved source or properly documented. [COS] All food items were VC&D and denatured with bleach.
