Observed accumulation of black mold-like substance in the interior panel of ice machine. [CA] Routinely clean and sanitize ice machine. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
- K41: Plumbing approved, installed, in good repair; proper backflow devices
Observed drain pipe from the 3-compartment sink and the coffee machine extend into the floor sink. [CA] Provide air gap between the drain pipes and floor sink that is at least twice the diameter of the inlet and no less than 1 inch.

Facility is currently operating without a valid permit. [CA] A food facility shall maintain a valid health permit during operations.

Measured quaternary ammonia concentration in the 3-compartment sink and sanitizer buckets at 100 ppm. [CA] Adjust sanitizer concentration. Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds, or as per manufacturer's specifications.
