    No paper towels at kitchen handwash station. No soap or paper towels in employee restroom (only bar soap). [CA] Provide liquid soap and single use paper towels in dispensers at all handwash stations. [SA] PIC provided a roll of paper towels and a bottle of pump soap during time of inspection.

    Observed open food containers in the reach-in cooler. [CA] Keep food protected with covers. Observed boxes of raw chicken, a bag of onions, and a bag of sugar on the floor. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

    Observed prepackaged food to not be labeled (mango lasse). OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: FALIFORNIA NEW OWNER: AHSMH, INC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/01/2024 - 11/30/2025. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
