OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: YOGURTLAND NEW OWNER: LINH NGO The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP9. An invoice for the permit fee in the amount of $815.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 02/01/2025 - 01/31/2026 This report serves as a temporary permit. A NOTICE WAS MAILED TO THE DEPARTMENT STATING: "BUSINESS WAS SOLD ESCROW CLOSE 6/20/24" FINDINGS: THE NEW OPERATOR WAS FOUND OPERATING WITHOUT A PERMIT. PLEASE REQUEST A CHANGE OF OWNERSHIP TO DEHWEB@DEH.SCCGOV.ORG GIVE THEM THE FACILITY ADDRESS AND CONTACT INFORMATION. They will send you a change of ownership application and a permit application. Please include the menu including any snacks. Once you have done that, the change of ownership paperwork will be able to get the process started. THE CHANGE OF OWNERSHIP MUST BE DONE WITHIN 48 HRS.
