Found damaged wall near oven. [CA] Seal up all holes, gaps and crevices to prevent entrance and harborage of vermin.

Found a few flies near warewash station in prep area. [CA] Each food facility shall be free of pest such as flies. Provide approved pest control services.

Found warewash sink (right side) with ONLY a spray nozzle, facility was about to wash dishes. Observed warewash sink not set up with wash, rinse and sanitize. [CA] Facility shall install a goose neck faucet to ensure proper warewashing procedure.

Found a fly landing on bread in display case at front self service line. [CA] Food is adulterated if it bears or contains any poisonous or deleterious substance that may render it impure or injurious to health and shall be discarded immediately. [COS] Operator voluntarily discarded bread.

Found flies in the following areas: back prep area and in bread display case at self-service area. [CA] Each food facility shall be free of pest such as flies. Provide approved pest control services.
