Wiping cloths were stored on prep surfaces throughout facility [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium).
- K30: Food storage: food storage containers identified
Several containers of food did not have labels and PIC was unsure what was in the containers. [CA] Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food if not easily recognizable.

Handwash sink blocked by cart. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Cart was moved to allow access to sink **REPEAT VIOLATION**
- K23: No rodents, insects, birds, or animals
Fruit flies throughout food prep areas and bathroom. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.

Excessive food debris build-up on floors and under equipment. [CA] Walls and/or floors in food preparation area shall be kept clean.

Excessive clutter and broken equipment all around kitchen and dry storage areas. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used to prevent the harborage of vermin.

1.Hot water at the 3-compartment sink measured 114F. 2. Hot water at the prep sink measured 110F. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks.
- K33: Nonfood contact surfaces clean
Refrigerator handles and sides of cupboards have food debris build-up. [CA] Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
- K05: Hands clean, properly washed; gloves used properly
Employee only used hand sanitizer without handwashing in between tasks. [CA] Properly wash hands with soap, warm water and dry using single use paper towels as required. Hand sanitizer must be approved and shall be applied only to hands that have been properly cleaned. [COS] Employee was instructed to wash hands properly.

Black mold-like substance on inside of ice machine. Not actively touching ice. [CA] Clean inside of ice machine. **REPEAT VIOLATION**

Cut tomato in prep insert measured at 47F. Per PIC tomato was placed in unit one hour prior to inspection. [CA] PHFs shall be held at 41°F or below [COS] Tomato was moved to 2 door upright cooler.
