Observed time marked on small white board passes the 4 hour mark, per employee food was placed in steam table at about half hour before inspection and forgot to change time; observed food temperatures above 135F. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] Time was changed to time food was added.

4 hand sink with a mixing valve have a water temperature is between 94 F - 122F. [CA] Handwashing facilities equipped with a mixing valve that is not readily adjustable at the faucet, shall provide warm water at least 100°F, but not greater than 108°F. Use other nearby sink to wash hands.
