    Restroom hand wash sink is out of liquid soap in dispenser and bar soap is available. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times.

    Observed vegetables and nuts out of original packaging and packed in small bags without proper labeling. Facility lacks a three compartment sink, prep sink and a hand wash sink set up for any food prep. Facility is not permitted to do any food prep. Observed more small ziplock bags of cut vegetables in display refrigerator. [CA] All food contact surfaces of utensils and equipment shall be cleaned and sanitized as required.     Water at hand wash sink is draining slow. [CA] Repair plumbing fixture and maintain in clean and good repair.

    Observed two door refrigerator with some produce items stored outside back area of the facility. [CA] Only prepackaged non potentially hazardous foods or uncut produce may be displayed or sold outdoors. Move the refrigerator inside the facility.

    Observed multiple holes in back screen door. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.
