ROUTINE INSPECTION
Notes: facility lacks a dedicated handwash sink in the food prep area. Wall-mounted paper towel and soap dispensers were set up at the 2-compartment sink. Upon remodel and/or change of ownership, facility will be required to install a handwash sink and 3-compartment sink.

Observed the faucet at the 2-compartment sink was leaking water when faucet was turned on. [CA] Repair faucet.
- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
Observed several (4+) upright refrigerator/freezer units were not in use in the rear storage area; some had nonfood items stored inside. [CA] Equipment shall be used for their intended purpose, or removed from the premises in order to reduce risk of vermin harborage. *REPEAT VIOLATION*

Inside a prep insert: measured internal temperature of sliced cheese to be 48-49F. Employee stated the cheese was moved from the walk-in refrigerator to the prep unit 30 minutes prior. [CA] Keep potentially hazardous foods at 41F and below or 135F and above.
- K23: No rodents, insects, birds, or animals
In 2 sticky traps under a 2-door units in the rear area: observed 2 large dead cockroaches. [CA] Keep facility clear of cockroaches. *REPEAT VIOLATION*
- K33: Nonfood contact surfaces clean
Observed heavy accumulation of food debris under refrigerator handles, especially the top lids of the prep unit. [CA] Keep surfaces clean.
- K01: Demonstration of knowledge; food safety certification
The manager's food safety certificate is expired. [CA] Obtain a valid manager's food safety certificate.
- K43: Toilet facilities: properly constructed, supplied, cleaned
The restroom door lacked a self-closing device. [CA] Install a self-closing device on the restroom door.
