- K06: Adequate handwash facilities supplied, accessibleCritical
Major violation: There was no paper towel supply on the front handwash station. [CA] Refill the wall-mounted paper towel dispenser immediately. The team leader refilled the dispenser (COS). Minor Violation: Clean utensils were store on the side of the handwash which does not have splash guard. [CA] If clean utensil are going to be stored in within two feet of the handwash station, splash guard must be installed on both sides of the sink to prevent contamination from splash.

Drainpipe from the ice machine was immersed inside the floor sink. [CA] To prevent possible contamination from backup of sewage, please provide at least one inch gap between the drainpipe and the floor sink.
- K30: Food storage: food storage containers identified
Many food products on boxes were stored directly on the floor or on milk crate back in the storage area. [CA] To prevent insect hiding and breading and for easy cleaning of the floor area, please store all food products at least six inches off the floor on an approved shelf or dunnage rack.

There is a gap between the screen door and the concrete floor in the back door. [CA] To prevent insect/rodent infestation, please install a weatherstripping on the screen door.
- K40: Wiping cloths: properly used, stored
A couple of wet/soiled wiping cloths were stored on the counters. [CA] Wet/soiled wiping cloths must be stored in a bucket that has standard sanitizer concentration in between use which is 200 ppm quaternary ammonia or 100 ppm chlorine (bleach).
