Notes: - Facility has ceased cooking food. The Cadco oven was still observed on premises. If unit is no longer required for operation remove. - Facility is still assembling, preparing, and reheating potentially hazardous foods (lox sandwiches, bacon sandwiches [precooked bacon], and prepackaged burritos. A permit upgrade to FP10 will be required.

Repeat Violation: In the back of house area the hand wash station was blocked off by a movable storage rack. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. Relocate the storage rack

Repeat Violation: The Environmental Health permit was not available. [CA] The health permit issued by this department shall be posted in a conspicuous place in the food facility.

Facility has a non ANSI accredited toaster oven and microwave. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation).

In the back of house area, near the dry storage, a plastic bag of dirty rags had numerous small flies. [CA] Ensure to maintain the facility free of animals, pests, and vermin.
