Observed handle of the scoop in direct contact with the corn flour in bulk foods container. [Corrective Action] Store scoops in such a way the handle does not touch food.

Floor grout observed worn down by the dough mixer. [Corrective Action] Maintain floors in good condition.

Observed expired chlorine test strips. [Corrective Action] Ensure test strips are not expired.

Observed paint peeling on the walls by the flour storage area and by the utensil storage area. Accumulation of dust and debris on the floor underneath peeled walls. [CA] Maintain walls and floors in good condition.

Employee food handler card is expired. [CA] All food handlers shall maintain a valid food handler card for their employment as a food handler.
