Discussed the following with PIC: -Possible exclusion measures: replacing door sweeps to reduce gaps below doors, drain covers. -Increasing pest control service frequency and asking technician to document recommendations.

- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
Observed gaps larger than 1/4 inch below multiple community center doors. [CA] Eliminate gaps greater than 1/4" to prevent the entrance and harborage of vermin. Replace door sweeps.

REPEAT VIOLATION: Mechanical warewash machine observed in disrepair. [CA] Repair warewash machine and maintain in good condition.

Water temperature measured 68F at the restroom hand wash sink. [CA] Hand wash sinks shall be equipped to provide minimum 100°F water under pressure for a minimum of 15 seconds.
- K39: Thermometers provided, accurate
Facility has 2 metal probe thermometers; however, thermometers are inoperable and appear to need batteries replaced. [CA] An operable, accurate, easily readable metal probe thermometer (accurate to +/- 2°F) that is designed to measure the temperature of thin masses shall be provided and readily available to accurately measure the temperatures of potentially hazardous foods.

REPEAT VIOLATION: 1) 1 dead cockroach observed on glue trap next to single door upright cooler. 2) 1 dead cockroach observed on glue trap in womens restroom. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. Thoroughly clean and sanitize areas of dead cockroaches. Monitor glue traps daily and replace as needed. [COS] Employee discarded glue traps. Note: Facility receives monthly pest control services. Reviewed pest control report dated 11/14/25, no activity reported.
