Facility is only fulfilling pizza orders for schools. - Facility's probe thermometer was verified to be within 2F. - Facility was filling out pizza temperature logs onsite for school deliveries.

    1. Repeat Violation: In the back of house area, near the soda syrups, the doors have been removed exposing the facility to an unfinished electrical panel area. 2. There is a square cut out in the wall under a prep table that has had a small metal grid attached to seal. 3. Cove base tiles near the walk in cooler are either broken, missing, or detaching from the cooler. 4. There is a gap around the soda syrup lines in the ceiling tile. [CA] Walls and/or floors in food preparation area shall be kept clean and in good repair. Repair the wall with a durable, smooth, nonabsorbent, and easily cleanable surface.

Note: rodent droppings were observed on top of the unused cooler stored under old beer kegs. Notes: - Per staff they wash produce using a colander in the rinse section of the three compartment sink and use the prep sink to pre rinse wares into the dishwasher, which is located under the drainboard of the prep sink. - The prep sink has a basin that does not meet the minimum requirements. The food preparation sink shall have a minimum dimension of 18 inches by 18 inches in length and width and 12 inches in depth with an integral drainboard or adjacent table at least 18 inches by 18 inches in length and width. The current sink is about 20 inches in length, 20 inches in width, and 5.5 inches in depth. Further evaluation required.

    Observed customers enter the bar area to access the self service soda machine. [CA] The "person in charge" shall ensure that persons unnecessary to the food facility operation shall not be allowed in the food preparation, food storage, or warewashing areas. Facility may clearly delineate the soda fountain area from the bar to prevent access into the bar area. 113984.1. [CA] All equipment that discharges liquid waste shall be drained by means of indirect waste pipes, and all wastes drained by them shall discharge through an airgap into a floor sink or other approved type of receptor.
