    Two types of tapioca pearls subjected to time as a public health control (TPHC) were not labeled. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. ** Repeat violation. Note: If violation is to be observed during the next routine inspection, usage of TPHC will be revoked. All PHF items will be subjected to temperature control only.

    Cases of food and utensils maintained on floor on top of cardboard. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

    Facility has added a dual waffle maker at the front service area to prepare hot slider sandwiches. Per employee, items were added onto the menu recently. Waffle maker was maintained at the front service area, not under any time of ventilation system. Ingredients of the slider does not include any raw PHF items. [CA] Provide approved exhaust hood ventilation over all cooking equipment as required to effectively remove cooking odors, smoke, steam, grease, heat, and vapors. - Relocate waffle makers under type II hood system in the kitchen. Mechanical exhaust ventilation shall be on while cooking is taking place.

    Hand soap dispenser at the hand wash station at the front service counter inoperable. Employee stated that the dispenser stopped working today and that hands were being washed at the hand sink by the three-compartment sink, less than 10 ft away. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times.
