OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Eurest @ Oracle B10 Cloud Cafe NEW OWNER: Compass Group USA Inc The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 01/01/2026 - 12/31/2027 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 12/08/2025 *Permit condition: Approval of the panini press, counter top steamer, and induction burner without an exhaust hood is conditional and are only to be used for precooked foods. If there is a future change in menu or operation, or the cooking system use causes any sanitation, ventilation or safety problem, the exhaust hood exemption may be revoked and an approved mechanical exhaust system must be installed, or the unit removed. Ventilation problems include, but are not limited to, excess heat in the facility that results in uncomfortable work conditions for employees. The exemption shall not be deemed to supersede any local building and fire code requirements. In the event that a future change of ownership occurs, the new owner shall be made aware of these conditions. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.

Accumulation of rodent droppings on the floor underneath/behind the grease trap, underneath the three compartment sink, underneath the mechanical dishwasher, and underneath the Cambro holding box. Most of the droppings observed to be old/dried. *Note: Rodent trap observed on the floor underneath the handwash sink. Facility is serviced monthly by Terminex. Pest control report provided for review. [CA] Clean and sanitize area of dead cockroaches or old droppings.
