FOLLOW-UP INSPECTION
FOLLOW UP INSPECTION FROM 7/15/25 AND 7/16/25 IN WHICH FRESH RODENT DROPPINGS WERE OBSERVED ON THE FLOOR UNDERNEATH STORAGE ROOM CABINET SHELVING AT THE FRONT LINE, FOOD PREP TABLE, ON THE FLOOR IN MIDDLE OF FOOD PREPARATION AREA AS WELL. FINDINGS: THE FOOD FACILITY IS DEVOID OF RODENT DROPPINGS. PER OPERATOR: THE EQUIPMENT, STORAGE, FOOD CONTACT SURFACES AND FLOORS WERE CLEANED AND SANITIZED. OK TO RE-OPEN. SECOND RE-INSPECTION FEE WAS PAID: CONFIRMATION NUMBER: 3902469755 1. WORKSHEET HAS BEEN RECEIVED FROM PERSON IN CHARGE. 2. A REPORT FROM A LICENSED PEST CONTROL COMPANY HAS BEEN RECEIVED AND REVIEWED. 3. ALL SANITATION AND FOOD SAFETY ITEMS ON THE WORKSHEET HAVE BEEN VERIFIED AND COMPLETED. 4. ALL PREVENTION MEASURES ON WORKSHEET HAVE BEEN VERIFIED AS COMPLETED.
FOLLOW-UP INSPECTION
Follow up inspection for routine inspection on 07/14/2025 regarding the following violations: -K06 Adequate handwash facilities supplied, accessible -K23 No rodents, insects, birds, or animals Specialist observations: -K06 Adequate handwash facilities supplied, accessible: CORRECTED -K23 No rodents, insects, birds, or animals: NOT corrected. See major violation above. Notes: -Permit Owner provided copy of pest control receipt from Ecolab for service on 07/15/2025. Report states no rodent activity and to install/replace door sweep. Door sweep has been replaced. -This is the facility's first follow up inspection. Subsequent follow-up inspections shall be billed at $298/hour, minimum one hour, during normal business hour, Monday - Friday 7:30 AM to 4:30 PM, and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. Facility shall remain closed due to evidence of rodent infestation. - Facility is to cease and desist all operations. - Facility is prohibited from all food service activities which include, but not limited to: preparation, service, handling, distribution, and sale. - Facility shall remain closed until vermin infestation is completely abated and authorization is provided by this Division. - Facility shall: - Provide pest control report from a licensed pest control company that indicates the type of control measures taken and that the facility has been treated. - Eradicate all live and dead rodents and insects from the facility. - Clean and sanitize the affected area(s) and equipment. - Dispose of all food items that have been adulterated/contaminated. - Seal holes (e.g. in walls and ceilings, repair coved base), and entryways (e.g. weather-stripping around doors); keep exterior doors closed; screen openable windows. - Eliminate harborage inside and outside of the facility - remove unused/cast-off equipment, contributory vegetation, etc.) - Eliminate food and water sources inside and outside the facility. - Properly store items (e.g. at least 6 inches above floor in sealed rodent-proof/insect-proof containers; organize overly packed and disorganized storage areas). - Properly store and dispose of garbage and other waste (e.g. bagged wastes, keep dumpster lids closed, frequent removal). - Facility is to remain closed until vermin infestation is completely abated. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied red placard. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. ** Please note that pest control treatments for rodents or insects may require a sufficient amount of contact time for these to be effective. Consult with pest control company for guidelines on eliminating harborage, entryways, and food/water sources.

Employees belongings on prep table. [CA] No person shall store clothing or personal effects in any area used for the storage and preparation of food or food contact surfaces.
Handwash sink was out of paper towels.[CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Paper towels were refilled using paper napkins.
Food Safety Certification was not available during inspection [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. Food handler Cards were not available during inspection. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.
Cold holding unit ambient temp measured at 47F, measured several PHFs in temperature danger zone. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] Moved all items from unit into walk-in cooler.
