    No air gap provided between ice machine drain line and floor sink. [CA]: Provide 1 inch air gap between the mentioned water line outlet and the flood level rim of a floor drain to prevent contamination that may be caused by backflow or back siphonage.

    1. *REPEAT VIOLATION* Under counter dish machine observed under drain board of the three compartment sink. Dish machine installed without plan submission to and approval from DEH Plan Check. Violation also noted on routine inspection report on 09/09/2025. 2. Previous ice maker (Snooker model# SK529) was replaced with a new ice maker (Snooker model# XC-1000T) without DEH Plan Check approval.     Lack of single-use paper towels at the handwash sink located in the restroom. [CA] All handwash sink shall be able to provide warm water, soap, and single-use paper towels for employees to properly wash their hands. [COS] Employee refilled paper towel dispenser.

    Drain line from equipment extends into the floor sink located behind the front counter. [CA] Provide a minimum of a 1 inch air gap between the end of equipment drain lines and the rim of the floor sink.

  • K07: Proper hot and cold holding temperatures

    Milk in reach-in cooler located behind the front counter is measured at 49-50F. [CA] Potentially hazardous foods when cold holding shall be held at or below 41F. Refrigerator door was observed not properly closed. The temperature in the unit started to decrease after door was closed. Per employee, the milk was placed into the unit less than 1 hour ago.

  • K48: Plan review

    An under counter dishwasher was observed under drain board of the three compartment sink. Dishwasher was not part of facility's plan and is unapproved. [CA] The addition of new equipment shall be submitted to this department's Plan Check for review and approval prior to installation and use. Dishwasher was not installed or connected at time of inspection. Facility may be subject to enforcement action if dishwasher is installed and in use without approval.
