Paper towel dispenser was not working in back preparation area. Paper towels were not available. [CA] Single-use sanitary towels shall be provided in dispensers at all times during operation. [SA] PIC provided a roll of paper towels next to handwash sink until dispenser could be repaired. MINOR- Observed pitcher of water in handwash sink. [CA] Ensure that handwash sink remains clear and available for use at all times during operation.

Observed leak in the drain pipe of the handwash sink in the first restroom. [CA] Repair plumbing fixture and maintain in clean and good repair.

Posted permit has expired 06/30/2025. [CA] Ensure the current Environmental Health permit is posted.

Food handler card was not available for employee working. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler available for review. A valid food handler card shall be provided within 30 days of after the date of hire.
