ROUTINE INSPECTION
OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Gong Cha NEW OWNER: TN Brewluxe LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP09(further evaluation of menu and food preparation to be conducted to verify permit category). An invoice for the permit fee in the amount of $625.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 02/01/2025 - 01/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 1/15/2025 *Permit condition: none *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.
Drain line at ice machine and food preparation sink lack 1" air gap[CA] Food preparation sink shall discharge liquid waste by means of indirect waste pipes into a floor sink or other approved type of receptor equipped with a minimum 1" air gap.
ROUTINE INSPECTION
Food managers not available for review upon request. 1 food handler card observed to be expired. [CA]Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire. Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times.
Handwash sink in back missing paper towels. 2nd handwash sink between front and back area available for use. [CA] Keep handwash sinks fully stocked at all times. [COS] Employee restocked sink with paper towel dispensers.
Boba observed to be labeled with time marking (11:00-3:30). No SOPs were available for review. When asked employee did not demonstrate knowledge of TPHC procedures. Per employee 3:30 is an estimate of when boba is used up and did not know what to do with boba is there was excess boba at the end of four hours. [CA] Provide written procedures and time marking for PHFs utilizing time as a public health control (TPHC). Ensure employees are trained and familiar with TPHC SOPs. [COS] Employees discarded boba at the end of time marking and made new time marking. Facility shall create SOPs for boba and post on site for employees.
