    At the front beverage prep area's handsink, observed: - A stack of plastic trays was obstructing the handsink - The paper towel dispenser was empty - There was no soap dispenser PIC stated they do not use this handsink. [CA] Handsinks must be unobstructed and stocked with paper towels and soap at all times. Handsinks must be available to make handwashing not only possible, but likely. [COS] The trays were removed. PIC placed a pump soap dispenser at the sink and refilled the paper towel dispenser. *2nd repeat violation*

    Observed 5-10 small flies in the rear kitchen/warewashing area. [CA] Keep facility clear of flies.

***Note: the temperature of the cookline handwash sink water fluctuated between 100 - 115F then settled back to approximately 105F. If the temperature of water provided to a handwashing sink is not readily adjustable at the faucet, the temperature of the water shall be at least 100°F, but not greater than 108°F. Ensure the temperature is able to be maintained between 100 - 108F so that it is not too hot to prevent employees from washing their hands.

    Under the ice machine: observed lack of one inch air gap between the drain pipe and top of the floor sink. [CA] Maintain a minimum one inch air gap between the drain pipes and the top of the floor sink.

    At the walk-in freezer door entrance: observed the floor directly in the door frame was in disrepair/not smooth. [CA] Repair the floor at the freezer door entrance. All floor surfaces must be smooth, durable, and easily cleanable.
