Found one ceiling panel not properly installed, causing a gap in the ceiling for an electrical wire.

Observed employee wash multiple pans and spoons without sanitizing.
[CA] - Manual sanitization shall be accomplished in the final sanitizing rinse by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds.

Inside right restroom, observed toilet tissue dispenser is broken. Exterior doors are stained.
[CA]- Toilet tissue shall be provided in a permanently installed dispenser at each toilet. Toilet facilities shall be maintained clean and in good repair.

Found two containers of raw chicken and raw goat being rinsed at the janitorial sink.
[CA] - Do not use janitorial sink for foods, use the approved food preparation sink.
*At time of inspection, observed water splashing off raw beef into the air and landing on the floor.

Observed two pans of cooked biryani rice stored ont he counter, measured between 120-123F. Per chef it was recently cooked
[CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool.

The 3-comparment warewash sink is in disrepair, water cannot be held in the sanitizing basin. Observed water leak from bottom of the sink into a filled bucket then onto the floor. A second bucket was placed in this compartment with bleach water, however it cannot fit most utensils/food-contact surfaces for adequate sanitizing.
[CA] - Liquid waste shall be disposed of through the approved plumbing system and shall discharge into the public sewerage or into an approved private sewage disposal system. Repair sink to ensure it can hold water. Eliminate any leaks
