The permit category for this facility is FP09 . An invoice for the permit fee in the amount of $ $652.00 will be mailed to the billing address on the application. Please be advised that a 25% permit fee penalty will apply for operating without a valid health permit. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 01/01/2026 - 12/31/2026. On site following correspondence received from previous owner stating this facility had a change of ownership in 2022. This facility is operating without a valid health permit. Health permits are non transferable. New owner is not onsite during this inspection. Owner requested for report to be emailed. A food facility shall not be open for business without a valid health permit. Facility will be subject to 100% penalty fees. Within 48 hours new owner shall submit the change of ownership food facility evaluation form and pay all applicable fees. Failure to complete process within 48 hours will result in facility being ordered closed for lack of valid health permit.
