    Received anonymous complaint stating that closure sign had been covered up. Complainant provided pictures. Upon the site visit conducted on 11/26, closure sign was observed to be covered up. During this follow-up inspection on 12/01/2025, tamper proof tape observed to have been tampered with. Owner states that he is aware of the requirement to not cover up the closure sign. Facility will be billed for covering up the closure sign. The following must be provided prior to reopening: 1. The signed and completed Reopening Checklist, emailed to the assigned inspector 2. PIC states that occasionally the sewage drain underneath the three compartment sink will back up owing to shared sewage lines with nearby apartments, and she thinks this is the source of the cockroaches. No sewage backup observed at time of inspection. Pest control report could not be provided. PIC is unsure if the property maintains a regular pest control service. 2.
