    Tub of raw onions maintained directly on the floor inside the walk-in refrigerator. Numerous bags of flour maintained on top of flatten cardboard box in the dining area. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

  • K08: Time as a public health control; procedures & recordsCritical

    Numerous cases of various types of steamed buns maintained out in ambient subjected to time as a public health control (TPHC) were not documented. Manager stated that buns were recently steamed and usually sell out within the 4 hours. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review.

  • K23: No rodents, insects, birds, or animals

    Two dead cockroaches observed on floor between the metal storage rack and chest freezer behind the burners. Manager stated that facility is serviced monthly. Pest control report was provided for review upon request. [CA] Clean and sanitize area of dead cockroaches.
