Facility is closed due to live cockroach activity in kitchen. Prior to re-opening, facility shall do the following: 1. Cockroach activity is to be eliminated 2. All contaminated affected areas are to be cleaned and sanitized 3. All contributing factors such as cleaning, repairs, and elimination of harborages 4.
