ROUTINE INSPECTION

PIC informed district inspector at the start of the inspection that the mechanical warewash was not in use and was not working. Hot water measured 120F. Per PIC, it was in the process of being repaired.

    Observed food stored on floor of walk in freezer. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination.

    Tofu measured 48F and macaroni salad measured 44F in self service area. Per PIC, they had been brought out from refrigeration 30 minutes prior to temperature measurement. All food is discarded at the end of service. [CA] PHFs shall be held at 41°F or below or at 135°F or above.

    Observed raw shelled eggs and pooled eggs stored above and next to ready to eat lunch meat items in the walk in fridge. [CA] All food shall be separated and protected from cross-contamination. Store all raw meat or other raw products of animal origin below ready-to-eat food items.

    Handwash sink in dishwashing area was not working at time of inspection. Soap and paper towels were not supplied. Per PIC, maintenance was aware of the issue and working on a repair. Second handwash located nearby. [CA] Ensure that soap, paper towels are provided at all times during operation and that handwash sink is available for use/maintained in good condition at all times.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: CulinArt Inc. @ Stryker Back of House NEW OWNER: CulinArt Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP14. An invoice for the permit fee in the amount of $1,723.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 01/01/2025 - 12/31/2025 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.     The faucet at the handwashing sink in the service area was observed loose. [Corrective Action] Ensure faucet is secured.

    Utensil surface temperature in the high temperature dishwasher was measured at 150 degrees Fahrenheit on the first pass through. [Corrective Action] Ensure utensil surface temperature reaches a minimum temperature of 160 degrees Fahrenheit on the first pass through for proper sanitizing.
