OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: T-Kila Place NEW OWNER: Ma Cristina Ruiz Gonzalez The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP09. An invoice for the permit fee in the amount of $652.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/01/25 - 11/30/26 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 11/14/25 *Permit condition: No Food Preparation is to be conducted at this facility where only cold holding and reheating for immediate service will be conducted. Foods are catered from a restaurant. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date. * Facility asked about installing an ice machine. Mon-Friday from 7:30-10:00 am there is a plan check senior inspector available. * Will operate Wednesday - Sunday from 4pm-1am.
ROUTINE INSPECTION
Failure to comply with repeat violations may result in further enforcement actions from this department such as additional inspections, including any fees associated with additional inspection time.
1. In the back of house area, there are several openings into the ceiling from either pipes or moved/misplaced ceiling tiles. 2. In the back of house area, at the turbo chef oven area, the wall has an accumulation of dust and grease. 3. In the back of house area, when the hand sink was turned on, an offensive odor was produced from the direction of the floor sink/waste pipe. [CA] Ensure to maintain the floors, walls, and ceilings of the facility clean and in good repair.
1. In the bar area there is a non-ANSI accredited mini cooler. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). 2. All the exhaust vents of the turbo chef oven are compacted with dust. [CA] Ensure to maintain all equipment clean and in good repair.
Facility lacks testing materials for their choice of sanitizer, chlorine. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.
1. MAJOR VIOLATION: Paper towel dispenser at the bar was empty. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Morgan restocked the paper towels. 2. MAJOR VIOLATION: There was no soap available in the restroom. A bottle of hand sanitizer was available. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. [COS] Morgan restocked the soap dispenser.
REPEAT VIOLATION: The restroom door does not self close. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.
In the back of house area, there is a under counter cooler stored above a functioning cooler that is in disrepair and no longer required for the operation of the facility. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.
In the back of house area, in space under the hand sink, an accumulation of rodent droppings were observed. [CA] Clean and sanitize area of old droppings.
REPEAT VIOLATION: A food safety certificate was not available upon request. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times.
In the back of house area, inside the working undercounter cooler, there is an accumulation of liquid waste. [CA] Ensure to maintain the non food contact surfaces of equipment clean.
