336 Routine OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: IKITCHENS NEW OWNER: Esha Corp The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP06. An invoice for the permit fee in the amount of $777.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 11/01/24 - 10/31/25. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 10/03/24. *Permit condition: Facility is permitted as a rental kitchen and shall ensure all food operations at this facility have valid environmental health permits and other required licenses/registrations for their operation.
FOLLOW-UP INSPECTION
This was a second follow-up for the inspections conducted on 8/14/24 and 8/19/24 at 336 Commercial St. No evidence of cockroaches were observed. PIC stated they had multiple pest control companies service the facility. Observed pest control reports dated 8/29/24 and 9/5/24. Continue to monitor for cockroaches and treat facility regularly. This inspection will be billed $298.00.
FOLLOW-UP INSPECTION
This was a follow up to the routine inspection conducted on 8/14/24. Observed at least 2 live (slow-moving) cockroaches. Due to the presence of cockroaches, another follow-up inspection will be conducted one week from today, 8/26/24. Contact Mindy.Nguyen@deh.sccgov.org if there are any changes or updates. The next follow-up will be charged $298. Failure to comply may result in enforcement action. Highly recommend having professional pest control perform at least one service before the next inspection.
Observed cockroaches in the following locations: - 1 live, slow-moving nymph cockroach on the floor in front of the mop sink by the water heater - 1 live, slow-moving nymph cockroach on the wall by the back door behind the dry storage shelves - 1 slow-moving (could not confirm live or dead) cockroach on the floor near the back door. [CA] Keep facility clear of cockroaches. [SA] Live cockroaches were killed. See notes below.
