    Observed fresh mouse in facility. Observed fresh rat droppings and numerous holes outside of the shack that lead directly inside. [CA] Completely eliminate rodents from facility. Patch holes and entry points. Recommend working with professional pest control.

    Potentially hazardous food located in a warmer measured in the temperature danger zone (hot dogs 118F). Per staff, the hot dogs were brought to the shack from the main kitchen around 11am. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Repair, adjust or replace unit if necessary. [COS] PIC had staff voluntarily discard the PHFs.
