1. The three door upright cooler near the back entrance is in disrepair. 2. The chest freezer is in disrepair. [CA] Ensure to maintain equipment clean and in good repair. Repair, replace, service, or remove equipment. 3. Facility is using cardboard to line shelves. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

Repeat Violation: The restroom door is not self-closing. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.

An open bag of beans was found in the storage room. [CA] Store open bulk foods in approved NSF containers with tight fitting lids.

Repeat Violation: There is a gap that is larger than 1/4' inch under the exit door of the "warehouse" storage room. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.
