OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Pho Ha Noi San Jose - Preparation Kitchen NEW OWNER: Hanoi Bistro Inc. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP14. An invoice for the permit fee in the amount of $1,723.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 06/09/2025 *** Note: - Per owner and employees, facility prepares food for other restaurant located outside of Santa Clara County. - Facility will be required to obtain Processed Food Registration (PFR) from California Department of Public Health (CDPH).

Facility is currently reduced-oxygen packaging PHF items without a HACCP plan. [CA] The food facility shall obtain a HACCP plan approval prior to packaging PHFs using a reduced-oxygen packaging (ROP) method and/or provide written standard operating procedures (SOPs) for ROP without HACCP approval. Note: - ROP activities at food facility do not require a HACCP plan if ALL the conditions below are satisfied: 1) Food is labeled with produce date AND time. 2) Food is held at 41F or below during refrigeration storage. 3) Food is removed from its package in the food facility within 48 hours after packaging.

Warm water at the hand was sink in the kitchen measured at 73F. [CA] Handwashing facilities equipped with a mixing valve that is not readily adjustable at the faucet, shall provide warm water at least 100°F, but not greater than 108°F.

Observed numerous non-certified equipment in use or stored in the facility: - Viet Dong vegetable slicer. - No-name brand vegetable slicer with conveyor belt. - No-name brand large vegetable recirculating washer. - No-name brand bag sealer. - 3x - no-name brand stainless steel induction stock pot stove with water faucet. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). Ambient temperature of the two-door upright refrigerator used to store creams and milk measured at 45F. [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below.

Old rodent dropping observed in the following areas: - On floor next to the two-door upright refrigerator storing milk and various types of creams directly outside of the kitchen. - On floor along the wall directly left once entering into the facility from the main entrance. No contamination of food or food contact surfaces observed. [CA] Clean and sanitize area of old droppings.

Observed heavy clutter of unused equipment stored inside facility. [CA] The premises of a food facility shall be free of litter and items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used.
