Notes: - Facility is closed due to lack of an adequate supply of hot water. Facility shall remain closed until hot water is restored and written authorization is issued from this department. - Replacement of the water heater will require plan check submittal and approval. The first follow up inspection is free of charge. Any and all subsequent follow up inspections shall be charged at the approved hourly rate of $298.00/hr, minimum of one hour, during normal business hours: Monday through Friday 7:30 AM to 4:30 PM, and $645 for a minimum of two hours, during non-business hours, and upon inspector availability.

[CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor. Note: Ensure to add pasteurization warning clause to juices. "WARNING: This product has not been pasteurized and therefore may contain harmful bacteria that can cause serious illness i nchildren, the elderly, and persons with weakened immune systems."

Facility shall close until hot water is restored and written authorization is issued from this department.
