The permit category for this facility is FP__13__. An invoice for the permit fee in the amount of $__1,453.00__ will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. 1. Two reach in refrigeration within the office are not NSF. Per PIC, they plan on removing the equipment. [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). 2. The mop rack is missing around the mop sink at the time of inspection. [CA] Install a new mop rack around the mop sink. 3. Caulking around the hood system is starting to fall apart and some gaps are appearing within the stainless steel. [CA] Remove old caulking and add fresh silicon within any gaps.

Damaged tile noted within the walk in and under the hood system. [CA] Replace broken floor tiles with a smooth and of durable construction and nonabsorbent material that is easily cleanable.
