A tray with bowl of flour was stored above trash bin. [CA] Food utensil (i.e. cutting board, baking pan, platters) shall not be used on top of a trash can to prevent cross contamination. [COS] PIC took the tray for washing. A bowl was stored above egg rolls at the prep cooler. [CA] Food utensil shall not be left on a food to prevent cross contamination. [COS] PIC picked up the bowl.

Drain pipe from the ice machine lack airgap. [CA] All equipment that discharges liquid waste shall be drained by means of indirect waste pipes, and all wastes drained by them shall discharge through an airgap into a floor sink or other approved type of receptor. Floor sink under the soda machine has a build up of a debris. [CA] Clean the floor sink.

Empty paper towel dispenser at the food prep area and employee restroom, hand washing sink. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels. [COS] Both Paper towel dispensers were refilled.

Apron was hanged inside the restroom. [CA] Employees can not hang apron inside the restroom. Apron shall be hang at a designated area.

[CA]Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. Food handler cards are not available for review. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.
