- K34: Warewash facilities: installed/maintained; test strips
Chlorine test strips (or other approved method) could not be provided for testing the sanitizer concentration of chlorine sanitizer during manual warewashing. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.
- K06: Adequate handwash facilities supplied, accessible
Hand wash sink at the middle of the preparation area observed obstructed by a trash can and a fan placed directly in front. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times. [COS] Obstructions were moved to the side.
- K33: Nonfood contact surfaces clean
Accumulation of brown buildup in floor sinks throughout the facility. [CA] Nonfood contact surfaces shall be maintained clean to prevent the attraction of vermin.
- K14: Food contact surfaces clean, sanitized
Employee observed spraying down dirty dishes/utensils with water, then placing them into sanitizing solution during manual warewashing, thereby skipping the "wash" step (using soap) of the manual warewashing process. [CA] Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM).

1. A valid food safety certificate could not be provided. [CA] Food facilities that prepare, handle, or serve non-prepackaged potentially hazard foods shall have a valid Food Safety Certificate available for review at all times. 2. Food handler cards could not be provided. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.
- K13: Food in good condition, safe, unadulterated
Observed 6 heavily dented cans of orange pulp (including denting along hermetic seal) maintained in a cardboard box on a rack in the dry storage room. Per the PIC, an employee had probably dropped the box of canned foods by accident. [CA] Canned food with denting along the hermetic seal shall be immediately removed from use. [COS] Dented cans of orange pulp were separated and labeled not to be used.
