FOLLOW-UP INSPECTION

Follow-up inspection for facility closed due to cockroach infestation. No evidence or signs of cockroaches activity was observed at time of inspection. - Facility has completed required items on reopening checklist. - Pest control service reports for 10/01/25 and 10/3/25 was reviewed that showed treatment was conducted. Service report from 10/3/25 notes no live activity. Per operator, the pest control services will continue at a frequency of twice a month. - All affected areas noted during previous inspections has been cleaned and sanitized. Per operator, the entire kitchen and dining area has also been cleaned and sanitized. - Previously opened foods stored in the facility have been discarded. - Gaps, crevices, and openings have been sealed or eliminated. Facility is OKAY to reopen. --------------------------------------------- Facility has change it’s dba from Milk Tea Lab and is now operating as Lo Si Ding Noodle. The current Environmental Health Permit is issued under the ownership of Milk Tea Lab SJ LLC. Operator shall provide evidence that the ownership of facility has not changed. If ownership has changed, facility is require to submit a facility evaluation application and permit application as the facility is operating without a valid permit. New permit will be subject to a 25% penalty fee. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees. ROUTINE INSPECTION

Limited inspection conducted in conjunction with complaint - CO0157987. On site investigation for a complaint of cockroaches in the facility. Upon arrival Specialist spoke with the person in charge who mentioned they are aware of cockroaches in the facility and have received pest control treatment. Note: Facility permit name is Milk Tea Lab and outdoor sign for facility is Lo Si Ding Noodle. 1. Observed Vermin: Documented in the following areas: - 1 live adult German cockroach behind the upright 2 door freezer. - 2 dead German cockroaches under food prep table near food storage. 2. Photographs: yes 3. Supervisor Notified: yes 4. Notification: The person in charge during inspection, Michelle Yeung, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. 3.
