    Appetizers observe to be kept out on trays at tables for customer self service. Food on display shall be protected from cross-contamination, by the use of an approved sneeze guard that intercept a direct line between the consumer's mouth and the food being displayed. Provide dispensing utensil for each food item offered at consumer self-service counter to prevent cross contamination. [COS] Operator will move food behind counter and employee will be handing food to customers.

    TPHC logs for appetizers observed to be not present in facility. Per operator TPHC logs are kept with runner who will be in charge of discarding and stocking food. [CA] Amend SOPs for PHFs to reflect procedures of logs being kept at a secondary location or keep TPHC logs with PHFs at facility in question. [COS] Log was able to be provided by food runner. Per log the food was placed out there in the last 30 minutes.

    Bruschetta observed to kept on countertop. Per operator items are on TPHC. No TPHC log observed at the bar [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] TPHC log with bruschetta was found in kitchen and a copy of sent to bar.
